Insulin resistance, explained without the jargon

It's the quiet engine behind so much of PMOS (PCOS) — and once you understand it, "just eat less" stops being the answer.

Insulin resistance gets thrown around in PMOS (PCOS) circles like everyone already knows what it means. Most people don't, because it's usually explained in textbook language. Here's the version that actually helps.

Start with what insulin does

When you eat, carbohydrates become glucose — sugar — in your blood. Insulin is the hormone that knocks on your cells' doors and says, "open up, take this glucose in for energy." In a body that's running smoothly, a little insulin gets a lot done.

With insulin resistance, the cells stop answering the door as easily. So your pancreas does the only thing it can: it sends more insulin. Blood sugar may still look normal on a basic test, because all that extra insulin is keeping it in line behind the scenes. That's why insulin resistance can go unnoticed for years — the standard glucose check looks fine while insulin itself is quietly running high.

Why that makes weight feel "stuck"

High insulin is, among other things, a fat-storage signal. When it's chronically elevated, your body is biased toward holding on to fat and away from releasing it — no matter how disciplined you are. This is the part that's so maddening for women with PMOS (PCOS): the effort is real, the willpower is real, and the scale still won't move, because the hormonal setting underneath is working against you.

It's also why so many women are handed "eat less, move more" and walk away feeling like a failure. The advice isn't wrong, exactly — it's just aimed at the wrong floor of the building.

The PMOS (PCOS) feedback loop

In PMOS (PCOS), high insulin doesn't stop at fat storage. It also pushes the ovaries to make more androgens — the hormones linked to irregular or missing periods, acne, and excess hair growth. Those symptoms, the weight, and the insulin all feed one another. Lower the insulin pressure and you often ease the whole loop, not just one symptom. That's the logic behind treatments — including GLP-1 medications — that improve how the body responds to insulin.

A note on testing. Fasting insulin, HbA1c, and ratios like glucose-to-insulin can give a fuller picture than a single glucose reading. What's worth ordering is a clinical decision — but it's reasonable to ask whether your provider is looking at insulin at all, not just blood sugar.
